Work Placement: Change4life by Sports & Leisure Management (SLM)

Job description

Everyone Active are carrying out community active sessions within the ashfield area visiting primary schools to deliver the Chang4life programme January 2017 - July 2017.

Change4life programme;

  • Will be delivered in Ashfield primary schools Jan-April and April-July in the afternoon and afterschool time
  • 12 weeks of physical activity and healthy eating
  • Fitness tests and healthy eating quizzes on week 1 and week 12
  • Parents may be invited to take part with their children
  • Children are encouraged to lead a healthy lifestyle and keep active for 60 minutes each day 

The programme manager is always looking for students/volunteers to help deliver the activities as there is a shortage within activity and leadership.

Areas will be: Selston, Huthwaite, Sutton, Kirkby and Hucknall. If you can travel to these locations please apply urgently.

Please note - Placements are open to all students at Vision West Nottinghamshire College, this may result into the placement being allocated; we do advise all students interested in the placement to speak with the employer in the first instance to check that the placement is still available.

Step 1 - Once you have a confirmed offer, in order to proceed with the work placement you will need to complete the work placement application form on Pro Portal, the form can be found under miscellaneous. (An accurate start and approximate end dates MUST be entered)

Step 2 - Your tutor will be asked to authorise your placement once you have fully completed your application form (remember to SAVE).

Step 3 - Once authorised, the work placement team will ensure the health and safety documentation for your placement is in place.

Step 4 - The work placement team will then send you, your tutor and the employer an email to confirm your work placement is ‘ Ready to Go’.

Step 5 – Collect a Progress Passport which you must complete on placement and ask your supervisor to sign off your hours and provide feedback on the last pages.

Step 6 – To check the status of your application, look at the note section at the bottom of your application form.

A parental consent form must also be signed and passed to your tutor before you go on placement.

We encourage you to secure your work placement as soon as possible. The deadline to have a placement agreed is Friday 8 Dec 2017.
